Do you accept insurance?

Balanced Nutrition LLC is currently in-network with:

  • Cigna

  • Regence

  • Premera

  • First Choice Health

  • Lifewise

 

A superbill can be provided for insurance companies that Balanced Nutrition LLC is out-of-network with.

Will my insurance cover an appointment when you are paneled?

It depends on your health insurance and reason for your visit. Most insurance companies cover nutrition services for eating disorders and diabetes. Some will also cover wellness nutrition services. To be sure your sessions are covered, please call your insurance company ahead of time. Important questions to ask them are:

  • Do I need prior authorization before nutrition services are rendered?

  • Is there a limit to the number of visits I am allowed?

  • Do I need a referral from my primary care physician for services?

  • What are my out-of-pocket expenses?

Initial nutrition assessments CPT code is 97802. Follow-up sessions CPT code is 97803. The most common ICD-10 code for nutrition services is Z71.3. This information may be helpful when contacting your insurance company to inquire about benefits surrounding nutrition services.

My insurance does not cover my nutrition appointments. Now what?

if you insurance company does not cover your nutrition sessions, you will be able to pay out-of-pocket for these services. Payment is due at the beginning of each appointment. Balanced Nutrition LLC accepts cash,  Visa, MasterCard, and Discover card. If you have a HSA (health savings account) or a deductible that must be met, you can request receipts and submit them for reimbursement.

What do I need to bring to my first appointment?

Be sure to bring your insurance card, any completed intake forms you have, a list of current medications and supplements, any recent lab work, list of your medical providers, and any food logs/journals you may have.

What is your cancellation policy?

Balanced Nutrition LLC requires at least 48 business hours notice for cancellations or rescheduling. This is to allow for the smoothest process for everyone involved. Nutrition counseling sessions are a time commitment and for cancellations with less than 48 business hours notice you will be charged 50% of the session fee.
